A page like this can be designed for your organization to accept registrations from the public. You can have as many different fields as you like. To retrieve the complete registration list, you simply log in to a separate secure page using an assigned ID and password. You can then open the file with MS Excel or any text editor.
 
Give it a try! Fill out the form and click 'Send'. Repeat if desired. You will then be given instructions on where and how to download the registration list.
